1. Print the registration form AND the waiver. The policies sheet should be printed and kept.

PSA Staff will be at the House of Soccer, located on the SE corner of Wyoming and Montgomery, for our registration weekend, Sept. 10th & 11th to accept registration forms in person and answer any questions you may have.
